Outreach Programs

Way@Work

Way@Work is a healthcare access enrollment program designed to streamline access to medical care for uninsured workers who qualify for free care before they get sick.  Enrolled workers receive a clinic care card that makes registration easier when an appointment for care is needed.  Enrolled companies can support the program through voluntary corporate contributions.

ClayPASS Care Coordination

ClayPASS is a countywide medical care coordination program designed to navigate uninsured and underinsured residents to appropriate outpatient care.  ClayPASS also diverts patients from using hospital emergency rooms and prevents costly inpatient readmissions by navigating discharges to free or affordable aftercare treatment.  The Way Clinic is an operational subscriber to the Florida Agency for Health Care Administration's Health Information Exchange, connecting the clinic to all medical providers in the region.

Way Mobile Medical

The Way Free Medical Clinic's mobile medical unit provides medical outreach to outlying areas of Clay County, bringing free quality medical care and health screening to rural residents who have transportation and other barriers to accessing care.

[Mobile deployments are scheduled to begin later in 2025.]

FoodRX

FoodRX is a comprehensive adjunct medical program focusing on "food as medicine."  Patients receive free healthy food distribution from The Way Clinic's onsite gardens, as well as food sourced from regional providers.  Patients also have access to healthy eating classes and culinary and horticultural education, as well as access to resources to develop their own healthy garden at home.
